HASI - Mental Health Requisition ID: req7669 Employment Type: Permanent Full-Time We have an exciting permanent Full-time Community Support Worker with our HASI plus team based in Chatswood. This position involves shift work, between Monday to Sunday, 24/7 rotating roster. New Horizons is a great place to work and grow. Key Responsibilities Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals- Regular reviews of progress. Collaboration with Community Mental health staff around clinical goals set for customer Complete comprehensive risk management plans for customer Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required Supporting customers to explore their practical and complex needs Keeping customer records up to date on a daily basis using online systems Developing and maintaining professional relationships and networks with customers, families, caregivers and key stakeholders Seek out innovative ways to enhance the customer journey Skill and Experience Knowledge of mental illness; preferable with experience working with people with complex mental illness You are experienced in the disability sector with an understanding of recovery recovery-orientated approach You'll also need Technical literacy with proven ability to use Microsoft Office and systems for reporting. Minimum Cert IV in Mental Health or Disability or equivalent industry experience Availability to work across a rotating roster Valid NSW Drivers C class licence, with the ability to drive vehicles in licence class, including large cars and vans.
