Mental Health Clinician

: Challenging and rewarding work environment Work for one of Australia's leading hospital groups Fixed term contract until 31/12/2024, with view to contact extension. About the Role Working with the team to provide responsive and comprehensive psychiatric assessment and treatment advice for older adults experiencing mild to moderate mental health concerns. To support the Primary Care Sector to meet the needs of older adults who do not qualify for tertiary mental health care and may not have access to alternative services. To support service development and evaluation. Note that the role will entail face to face visits to our various LGA locations. We are located in Kew area with expected satellite offices to north and east. Assessment, diagnosis and treatment of older persons with mental health concerns Utilise evidence-based practice in care provision with the multidisciplinary team Clinical supervision and support of clinicians and trainees within the service. Liaison with primary health and community services Referral to tertiary Mental Health services as required Meeting reporting and quality targets Active contribution to capacity building, service development and research. Classifications for this position will range from: OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($86,304 - $99,210 per annum) Psychologist Grade 2 PK1 - PK4 ($90,812 - $102,757 per annum) About the Healthy Ageing Service The Healthy Ageing Service (HAS) is a collaboration between St Vincent's Hospital Melbourne and the Eastern Melbourne Primary Health Network (EMPHN). HAS delivers mental health services to older adults with, or at risk of experiencing mild to moderate mental health concerns via an Outreach Brief Intervention Model of Care. HAS is a prevention/early intervention service with a strong focus on Health Promotion for consumers living at home or in Residential Aged Care Facilities (RACFs). The Local Government Areas (LGA) serviced include Boroondara, Banyule, Nillumbik, Whittlesea, Manningham, Whitehorse, Knox, Maroondah, Monash, and parts of Yarra Ranges, Murrindindi and Mitchell. HAS aims to: Improve the integration of primary care and specialist mental health services. Improve the coordination of treatment for older adults with mild to moderate mental health concerns. Support better physical health outcomes for older adults. Support the Primary Care Sector to provide care through case collaboration, Secondary Consultation and mental health education. Improve mental health engagement in a vulnerable population, especially those experiencing discrimination, are disengaged or neglecting their healthcare needs.
